Description of Diabetes HomeCare Center The Diabetes HomeCare Center strives to make diabetes management more effective and efficient. The HomeCare Center achieves these goals by providing a link between the person with diabetes and their doctor to promote intensive diabetes monitoring. An important factor for people trying to achieve glycemic control is not only frequently measuring their blood sugar or blood glucose level but also making this information available for analysis and feedback. How Does It Work? The Patient Using a computer with Internet access, the person with diabetes can access the HomeCare Center and manually enter his or her blood glucose monitoring results. The HomeCare Desktop can also be used to transfer data directly from selected blood glucose meters directly to the HomeCare Center. If you are on the road or away from your computer you can use your cell phone (a cell phone that is equipped with a web browser) to access the HomeCare Center to record your monitoring results as well as view weekly and monthly statistical averages. Also, currently under development is a device called the HomeCare Link that will connect to your Blood Glucose Meter and download the medical information through the phone line directly to the HomeCare Center's Web site with the touch of just one button. The HomeCare Center The HomeCare Center provides an online source to manage and track a person with diabetes blood glucose monitoring information. After the data has been entered, the person and/or their authorized physician can view it at the HomeCare Center Web site. The HomeCare Center records the data in a traditional logbook format and displays the data in various statistical graphs and charts. The HomeCare Center is secure, reliable, and committed to providing excellent diabetes care. The Physician Allowing your physician to access your results completes the link between the patient and doctor. This will allow the doctor to monitor your status at any time. After viewing the information that the person with diabetes has submitted, the physician can communicate with the person via email regarding concerns or to recommend a change in the person's treatment. The HomeCare Center helps the physician provide better for the person with diabetes.
